49-Year-Old, One Year After High SMAS Facelift and Deep Plane Neck Lift
This 49-year-old woman was photographed over one year after undergoing a high SMAS facelift and deep plane neck lift. The high SMAS facelift utilizes the same sub-SMAS dissection as a deep plane lift but offers more powerful skin management in select patients—allowing for refined repositioning and improved tension distribution for natural, long-lasting results.
Her deep neck lift included platysmaplasty, submandibular gland reduction, and reduction of the anterior belly of the digastric muscle, providing a smoother, more contoured neckline.
Following her initial surgery, the patient achieved intentional weight loss, which resulted in mild residual neck skin looseness. To refine her results, she later underwent a minor enhancement procedure, achieving the elegant, defined contours demonstrated here.

This 56-year-old woman was photographed nine months after undergoing a deep plane facelift and deep plane neck lift, complemented with anatomic micro and nano fat grafting for volume restoration and contour refinement.
At the patient’s request, the upper third of the face is not shown; however, her complete rejuvenation also included upper and lower blepharoplasty and an endoscopic brow lift.
The combination of deep structural lifting and precision fat grafting allowed for comprehensive facial rejuvenation—enhancing contour, restoring youthful fullness, and achieving a natural, balanced appearance that maintains harmony across all facial planes.

Patient Story: This patient is a nurse who is employed in the same practice as Dr. Weinfeld in Austin, Texas. While she does not recall a specific trauma, she had several misshapen elements to her external nose and severely deviated septum. Her goals related to both functional change and appearance change. She underwent a complex rhinoplasty. Do to complete derangement of her septal cartilage it did not provide the grafting material that was required for restructuring of her nose. Thus, rib cartilage was harvested to help provide additional material to make the required grafts. Additionally, an absorbable structural sheet was used to provide internal support as the tissues healed. She has been very pleased with her improved form and function.

The patient has been very happy with her results and was surprised and pleased by the fact that the discomfort was significantly less than she expected. She only required prescribed medications for discomfort for one day.

Patient Story: This patient is woman from Central Texas with the primary cosmetic concern being her significant dorsal hump. The narrowness of her bridge and its projection was problematic and had negative ramifications in the appearance of the remainder of her face. For example in the pre-operative photographs one sees that her eyes appear to be too close together. She underwent a rhinoplasty with normalization of her dorsal height and width. The pre-operative “closeness” of her eyes is an optical illusion of sorts that appears to be “undone” when a well-executed advanced rhinoplasty is performed. This is another example of the fact that the nose is the aesthetic bulls-eye of the face. Its appearance affects the appearance of all of the other facial features.

Patient Story:
This patient is a woman in her late fifties who is the wife of a senior hospital administrator in Austin, Texas. She was interested in comprehensive cosmetic facial rejuvenation. Following her consultation with Plastic Surgeon Dr. Weinfeld, she felt the best approach for her was two-stage process. First she underwent an upper and lower blepharoplasty. The lower blepharoplasty was performed with a subciliary approach permitting septal reset and blending of lid cheek junction. This advanced approach provides more than just lower lid smoothing. It permits elevation of the upper cheek, softening of the nasojugal crease, and a decreased vertical dimension of the lower eyelid – all features associated with youth and attractiveness. Following three months she underwent and facelift and necklift. In the majority of patients, but especially in patients with full faces and necks, Dr. Weinfeld choices to use direct sculpting of the lower face and neck fat instead of using liposuction which can be less precise. The powerful effect of direct sculpting can be seen in her post-operative photos.
